Application form to attend June 16th and 17th Live Model Showcase

• The majority of the agencies are searching for High fashion talent between the ages of 12 and 25 yet some attending agencies will also be looking for commercial & lifestyle models (between ages 25 and 36). For this a woman should be between 5’7” and 6’0” tall and men between 5’10” and 6’2”. For commercial modeling you can be shorter than these requirements if you have a great look.

• If you are under the age of 18, we must speak with your parent/guardian before you can be considered as a participant to our Live Model Showcase.

Scouted attendees will be able to register online & pay for hotel and event.

If you have not been scouted by us you will be able to apply and send us payment information but we will need to interview you and run you through our scouting process to confirm your eligibility to attend the event.

By submitting this form you have read and agreed to the FMI Terms and Conditions. Once you have paid for your spot in our showcase we can not give a refund for that event. If you can’t make the December 11th showcase, we will allow you to attend our next live showcase at half price. After you have paid for the showcase, you will be receiving a confirmation letter from us along with further details regarding the event. Please allow 3-4 weeks to receive this package.
